Los afectados por el cierre de iDental podrán solicitar copia de su historial clínico
Los afectados por el cierre de las clínicas de iDental en Andalucía podrán solicitar, a partir del mes de octubre, copia de su historial clínico y de la documentación administrativa que contenga el expediente de iDental que ahora custodia la Consejería de Salud a través del Servicio Andaluz de Salud.
Los afectados por el cierre de iDental podrán solicitar copia de su historial clínico a partir de octubre al SAS.
El formulario para pedir esta documentación se podrá descargar en la web de la Consejería de Salud, el Servicio Andaluz de Salud y el servicio Consumo Responde.
Este formulario deberá entregarse preferiblemente en los centros cabeceras de Distritos Sanitarios y Áreas de Gestión Sanitaria, cuyas direcciones aparecen en el propio formulario.
Una vez recepcionada la solicitud, se remitirá al servicio de Atención a la Ciudadanía de los distintos centros hospitalarios que custodian la documentación.
La Dirección General de Consumo ha recibido y tramitado más de 4.100 reclamaciones relacionadas con las seis clínicas de IDental en Andalucía.

Those affected by the closure of the iDental clinics in Andalusia will be able to request, as of October, a copy of their clinical history and of the administrative documentation that contains the iDental file that is now kept by the Ministry of Health through the Andalusian Service of health.
Those affected by the closure of iDental may request a copy of their clinical history from October to the SAS.
The form to request this documentation can be downloaded on the website of the Ministry of Health, the Andalusian Health Service and the Consumer Respond service.
This form should be delivered preferably in the head offices of Sanitary Districts and Sanitary Management Areas, whose addresses appear in the form itself.
Once the request has been received, it will be sent to the Citizenship Service of the different hospital centers that keep the documentation.
The Directorate General of Consumer Affairs has received and processed more than 4,100 claims related to the six IDental clinics in Andalusia.
